Web Site Photo Release FORM

Christ the Cornerstone Academy is utilizing the Internet and the latest technologies through the development of its school web site at ctc-academy.org to streamline communications and support student learning. The CTCA web site is primarily used to publish school and related information for new, current and prospective students and their families. As such, student photographs and work may be included to highlight school & class projects and activities.

CTCA would like to ensure that certain web publishing guidelines are being followed in the development of the school web site. For this reason, we require parental or guardian consent for the release of student photographs and student work to be published at the CTCA web site.

Web publishing guidelines include:

The student's full name and other personal information such as address & telephone number will NOT be published on the Internet.

Student's web pages created as part of the school project may be published at the CTCA web site as appropriate.

Photos of large groups of students may be published on a school web site WITHOUT parental or guardian permission only if individual students are hard to identify even with full frontal view or one-half or more of the student's face is out of view.

If an individual student is identified in a photo, a photo release form to indicate that CTCA has NOT been granted the permission to publish any of the student's photograph must be submitted to the CTCA Office.

The identification of students in photos and identification of class or school work will be by initials only.

Unless a written statement to the contrary is filed with the Office of CTCA, all students, parents, and guardians give implicit permission and authorization to CTCA to use any photograph of themselves that is taken or is authorized by a CTCA staff member for instructional or promotional purposes. By granting permission, students and their families and guardians release any and all claims against CTCA.
