Enrollment Procedure
The prospective child and his/her parents will visit the school for an interview with the
Principal or Preschool Director. They may at that time visit the classroom and meet the teacher(s).
The following forms must be completed before a student may start attendance:
- CTC Enrollment Application
- Child's Pre-Admission Health History Parent's Report
- Physician's Report
- Identification and Emergency Information
- Consent for Medical Treatment
- Immunization Records
- Report of Health Check-up for School Entry (1st Grade)
- Notification of Parent's Right
- Personal Rights
- CTC Student & Family Handbook sign-off
- Child Abuse Reporting Booklet sign-off
- Scrip support sign-off
- Volunteer support sign-off
- Directory sign-off
Acceptance and grade level performance in day school may be based on an acceptable score
on a previously administered standardized achievement test or an entrance examination
prepared by our staff. If a child is applying to enter a grade other than age appropriate,
a letter of recommendation must be received from their former school. A student may
be evaluated in classroom participation and the teacher will then recommend proper grade
placement.
